Requirements
- Interested candidates must have earned a minimum of two (2) 5-credit passes in English language, mathematics, physics, chemistry, and biology.
- Combining WAEC with NECO or GCE is acceptable. To get forms, the applicant must be at least 17 years old.
How to Apply for Bayelsa State School of Nursing and Midwifery Admission Form
The sale of forms began on March 28th of 2025. Pay the non-refundable fee of N10,000 (ten thousand naira only) into Zenith Bank, account number 1013876303. Name of the account: Min Of Health, Sub Account.
Bring the original and a duplicate of the bank teller’s receipt to the Department of Nursing, located in rooms 18 and 20 on the third floor of the Ministry of Health’s office complex in Yenagoa, to pick up the entrance application.
Submitting of Completed Forms: Before July 8th 2025, all completed forms, along with any pertinent photocopies of documents, should be returned to the Department of Nursing, Ministry of Health.
