- Photocopy of admission letter and acceptance of the offer of admission
- Original commitment form
- Verified Statement of result or Certificate for O’Level
- Original JAMB Result with Passport
- Photocopy of JAMB Online Admission Letter – Institution copy
- Original course form
- Photocopy of Screening Teller
- Original permission from HOD
- Original medical certificate
- Photocopy of State of Origin certificate
- Photocopy of age declaration or birth certificate
- Two copies of the Certificate of Schedule and Confirmation of School Fees
- Photocopy of RRR and confirmation of all payments
- Photocopy of oath
