    • The Federal Polytechnic, Ado-Ekiti (FPA) Post UTME Screening result slip
    • Online Application form
    • Original copy of O’Level Certificate/Statement of O’Level result (Signed &Stamped)
    • JAMB UTME score printout
    • Change of institution/ course to FEDPOLYADO as a choice institution (If applicable).
    • Admission status print-out (via JAMB CAPS/Access my CAPS/Admission Status)
    • JAMB admission letter
    • FPA admission letter
    • My Biodata printout (via JAMB CAPS)
    • Six (6) recent passport photographs of normal size and four (4) of size 4 X 4
    • Seven (7) photocopies of each of the above are arranged in the set order.
    • Come with your nose mask(s) and be prepared to abide by given COVID-19 protocols.

    • One (1) year I. T. letter for candidates with grades of Distinction, Upper Credit and Lower Credit, respectively.
    • Two (2)  years I. T. letter for candidates with Pass grades.
    • Admission letter.
    • Six (6) recent passport photographs of normal size and four (4) of size 4 X 4.
    • Ensure that transcripts of Academic Records have been sent to this Institution (for candidates with ND from other Institutions).
    • Seven (7) photocopies of each credential above are arranged in the set order.
    • Come with your nose mask(s) and be prepared to abide by given COVID-19 protocols.
